ICC 2004 -- Conference Program : Optional forms

1.) Audio Visual

A speaker rehearsal room with LCD projector will be available Sunday - Wednesday from 7:30 - 18:00 for any
presenters who wish to rehearse and prepare for their presentations.

The speaker rehearsal room is located in the New York Hotel Convention Center.

ICC 2004 strongly advises all speakers to test out their presentation in the speaker's rehearsal room prior to their session.

The technical session rooms' LCD Projector is to be used with the speaker laptop. ICC 2004 will not be providing computers in session rooms.

2.) Student Travel Grant- PDF

A limited number of Student Travel Grants are available for highly qualified individuals.
W-8 Form (this form is required only if you receive a grant)

3) Overlength Form

If your paper is more than 5 pages you are required to fill out this form and send in payment

4) Reprint Form

If you are interested in receiving reprints of your paper you need to fill out this form and send in payment
